We have group homes in Chandler, Mesa, and Phoenix.
All our group homes have video surveillance.
Yes, Nursing support is scheduled in this group home on a 24/7 basis.

You can get your Fingerprint Card online through the AZDPS Website and you will have to make an appointment to go somewhere to have your prints rolled.


You can also go to HeartSavers in Mesa. They will take your fingerprints for $15.00 and give you the application and pre-addressed envelope.You will need to get a $67 money order and mail that with the completed application to the address on the envelope.
The box you will check off is, DES/DDD-HCBS.
